Fake news: Can it be fixed?

While they both have historical precedents, they've become insidious and difficult to stop. Why is that? Can anything be done? I'm interested to hear from people about how they see the problem, and together work on a potential solution in an off-the-record, Chatham House Rules setting.
Who is this event for?
